Overview
This is when a child or young person is moving between key phases of education.
This process is often called a "phase transfer". This wording may be used in Education, Health and Care Plan (EHCP) reviews.
Parent carers and young people should start thinking about the transition at least 18 months before it will happen.
If a child or young person has an EHCP, parent carers must give their preferences to the SEN Team. This should be done through the EHCP review process. It can also be done through an EHC needs assessment if a child or young person is in a transfer year.
It's advised that parent carers should also completed the school admissions form. The SEN Team will cross reference the preferences. Please be aware that independent specialist settings are not on the school admissions form.
