Some children and young people might need more help and support than others to live, healthy, happy lives.

You can go to your pharmacist for medical advice and treatments for things like colds, tummy troubles, rashes and pains, and information about prescriptions.

If you have a concern about your child's development, please talk to people who will be able to help. 

If you are a professional who has concerns about a child's development (0-5), you can complete the Health notification form (Section 23) form.

Who to talk to first

  • For babies and children 0 to 5 Contact your health visitor, doctor (GP), nursery, or the Early Years SEND and Portage Team for support and early intervention, ensuring children receive the right support at the right time and place.
  • For school-aged children and young people Talk to the school nurse, doctor, or school SENCO for guidance and referrals.
  • For young people over 16: Speak to the school nurse, doctor, or the educational setting’s SENCO for necessary support and referrals.

Family Hubs

There are five Family Hub areas in Calderdale which join up locally in the planning and delivery of family services from conception to young people up to 19 years or up to 25 years for those with special educational needs and disabilities (SEND).

Calderdale Cares Partnership are responsible for arranging and delivering health and care services for the 206,600 people who live in Calderdale.
